A massive fall of roof occurred in this mine while a split was being initiated in a pillar between road No. 569 and road No. 570 of the No. 568 road section.

A miner operator, miner-operator s helper, and a roof bolter machine operator were killed instantly.  The roof-bolting machine operator s helper was injured slightly.
